Dr. Chloe Cordell Hammett, DPT

Chloe is a Doctor of Physical Therapy with advanced training across musculoskeletal, neurological, and cardiopulmonary care. Originally from Oklahoma, she moved to Lafayette to play volleyball at the University of Louisiana before earning her DPT from Franciscan Missionaries of Our Lady University. She also holds a Master of Health Administration, reflecting her passion for elevating patient care and experience.

Chloe became invested in pelvic floor health after watching friends and family navigate pregnancy and postpartum recovery. Seeing a major gap in women’s health education inspired her to help create better access, understanding, and support. Her approach empowers patients through movement, education, and confidence, helping them take an active role in their healing.

Jordan Delany, PTA

Jordan has been part of Acadiana Pain & Performance Rehab since its early days, first joining the team in 2014 before later returning as a licensed Physical Therapist Assistant. She has advanced training in orthopedic manual therapy and sports rehabilitation, with a strong focus on hands-on care for the spine and joints.

Jordan specializes in improving mobility, reducing pain, and supporting recovery through manual therapy and full-body stretching, including leading the clinic’s Stretch Lab. She was drawn to pelvic floor therapy after recognizing how often women’s health concerns are overlooked or misunderstood. Her goal is to help change that through compassionate care, education, and practical guidance.
